Main Street Dental Staff Agreement
We will use the information you provide to see if MSD is able to treat your dental problem.
We may use your name and address to maybe send you newsletter about our practice and services we offer as well as any special promotions we may have running.
We do not disclose your protected health information for other marketing activities.
